How do I schedule a medical telemedicine appointment?
You may schedule a telemedicine appointment by completing our Book Your Appointment online form. In order to book, answer some simple questions about your health and select your appointment time. You will receive an e-mail with appointment details and a link to a secured video chat after the form had been submitted.
When should I expect a reply to my appointment request?
You should generally expect an e-mail reply within a couple of hours after you submit your request.
Can I use mobile device for my video consultation?
Yes, you may use your smartphone that is connected to Internet to get care from a licensed health care provider. You may also use your tablet or a desktop computer.
Can Evivia Clinic doctor refill or order prescriptions?
If the doctor deems it medically necessary to prescribe a medication, then they are able to do so. If the physician deems it necessary, medication refills are also available.
How do I get a prescription for a medication?
If you feel like you need to get a medication for your condition, you need to book your appointment via our online patient forms. You should receive an e-mail for a link to a secured video chat with a licensed medical provider. On the day of your appointment, follow the link to complete your online visit. After a one-one one consultation, your medical provider might prescribe you a medication and send it to a pharmacy of your choice, if he or she deems it necessary.
How will I receive my medications?
You will be able to choose which pharmacy you want to pick up your prescription from during your Telehealth visit.
What other medical services do you provide?

What is a Merchant Mariner Physical Examination?
Merchant Mariner Physical Examination is a fitness for duty medical exam performed by a U.S. licensed medical doctor, U.S. licensed physician assistant, or U.S. licensed nurse practitioner. It typically would include an evaluation of hearing, vision, and physical abilities as well as a documentary review of the applicant’s medical history and medications.
What is a Merchant Mariner Medical Certificate?
A Merchant Mariner Medical Certificate is a document that certifies that the seafarer fulfills the necessary medical and physical requirements and is capable of performing essential and emergency duties onboard safely and effectively.
What do I need to qualify for a medical certificate?
To qualify for a medical certificate, mariner applicants must provide evidence of meeting the medical and physical standards on the Application for Medical Certificate, Form CG-719K or the Application for Medical Certificate, Short Form CG-719K/E
Do I need to disclose over the counter medications and/or medications prescribed by my doctor?
Applicants should disclose all prescription medications filled or taken within 30 days prior to the applicant signs the application including dietary supplements and vitamins filled or taken within 30 days prior to the applicant signs the application.
What if I do not meet the physical, vision, hearing or general medical examination requirements to get the MMC Medical Certificate?
Not meeting vision, hearing, or general medical examination requirements does not automatically result in denial of your application for the Medical Certificate. In certain circumstances, the USCG might consider you for a Medical Waiver and request your doctor to provide additional medical information to support an issuance of a waiver.
What is a Medical Waiver (MV)?
The medical waiver is an acknowledgement by the National Maritime Center that medical condition was identified as not posing a significant risk to safety. The waiver places a responsibility on the mariner to report any change in the identified condition. Medical waivers may have specific requirements, such as more frequent medical monitoring of the medical condition, submissions of medical exams or tests to track the ongoing status of the condition.

Why did I get a letter requesting to provide supporting medical documentation?
Frequently, a mariner’s medical examination will indicate treatment for an illness, injury, or prescription medication which requires further clarification in order to make a fitness for medical certification determination. Awaiting information (AI) refers to additional information requested by the medical evaluation staff to complete the medical evaluation.
What is an Additional Information (AI) letter?
An AI letter requests additional information from the mariner in order to clarify a medical diagnosis, condition, or specific medication use. The National Maritime Center (NMC) may request further documentation to elaborate and explain medical history, current status report, diagnostic testing, etc., to obtain a more precise picture of the mariner’s health and fitness.
What are the medical standards for Merchant Marine Medical Certificate?
The standards for merchant mariner medical certification include requirements for vision, hearing, general medical examination and demonstration of physical ability.

What is your cancellation policy?
Please kindly know that appointments are in high demand, so when you book your appointment, you are holding a space on our calendar that is no longer available to other clients. Any cancellation of the appointment with less than 24 hours notice will result in a charge for a full price for the visit. A full price for the visit will be charged for a no-show. Our Custom Travel Document ($35) is a document that is prepared on an clinic’s official letterhead and will include the following information, depending on request and current requirements by authorities:
— Your sample collection date and time
— Your name and date of birth
— Type of testing (Molecular COVID-19 RT-PCR, Molecular COVID-19 PCR test, Rapid Antigen Test)
— Type of specimen collected (Nasal Swab or Nasopharyngeal Swab)
— Your passport number or Residence Card number (in case your travel destination requires it. Ex: Spain, Hong Kong, Thailand, French Polynesia, Dubai, Japan, Russia, Luxembourg, the Netherlands, etc. )

How can I pay for COVID-19 PCR test?
Your credit card information will be securely collected when you book your COVID-19 test appointment on our website. The charge will be placed once you submit your appointment request. You will receive an email with a receipt.
Do you administer COVID tests to children under 18?
Yes! Please make an appointment for your child on our website.

What is the difference between Molecular / PCR, Antigen, and Antibody Coronavirus tests?
There are three different types of tests for Coronavirus that are available today – two diagnostic and one Antibody test.
- Molecular / PCR aka NAAT, viral RT-PCR test is an accurate, most reliable technology that is considered to be the “gold standard” diagnostic test that shows an active Coronavirus infection. They are very sensitive and are highly specific.
- Antigen tests is a much simpler and faster test. It is less sensitive than that PCR Test, and there needs to be more virus present before the test can show positive. The Antigen test should be used within 5 days on symptom onset, when the viral load is typically very high.
- An Antibody test is a blood test that is used to determine if you had the the Coronavirus disease in the past. In other words, it looks for the body’s response to Coronavirus that causes COVID-19. This test is never used to determine is if you have the COVID-19 disease.
What kind of COVID-19 tests are available through Evivia?
Depending on your need, you may be recommended the following Coronavirus testing solutions that you may select or combine:
- RT-PCR (molecular) COVID-19 Swab Test. An FDA Authorized molecular in vitro diagnostic test resulting in an automated SARS-CoV-2 RT-PCR Assay.
This type of testing would be suitable for people who wish to travel abroad to countries that require an RT-PCR Test (ex: Spain, the Bahamas, French Polynesia, Nicaragua, Sint Maarten (Dutch Side).
- PCR (NAAT) COVID-19 Test that is tested using Abbot’s ID Now Instrument, a portable, FDA Authorized equipment and ID Now COVID-19 test. Results are available in hours. (Coming in December, 2020). Results within hours, same day results or results in 24 hours, based on your needs.
Rapid, FDA authorized under the Emergency Use molecular Antigen nasal swab test designed to detect COVID-19 disease.
Recommended for persons or businesses that need to get their Coronavirus results fast. This type of test would be most suitable for people or businesses that need periodic testing for Coronavirus or for some international and domestic travel destinations. - Rapid Antigen COVID-19 Test. How is the COVID-19 Test administered?
We have two types of swabs that we can used – nasal and a nasopharyngeal. The type of swab that will be used will be determined by our health care provider based on the tests that will be administered.

We will email you your COVID-19 test result, along with any documentation, via our secured e-mail. Please check your your spam folder if you did not receive an e-mail from us within the promised timeframe.
Do you guarantee that I will be admitted to my flight and destination country?
We are familiar with each country’s specific COVID-19 entry rules, restrictions and travel advisory and our documentation has been successful for all of our clients, but we cannot guarantee that you will be admitted by an airline, country, or customs personnel. We do our best to stay updated on each destination, however, it is the traveler’s duty to know the entry requirements.
What kind of testing equipment do you use for your testing for Coronavirus disease?
Abbot ID Now is an instrument that is used in our office PCR testing. It is an FDA Authorized Point of Care (POC) PCR, NAAT test created by Abbott to detect COVID-19/Coronavirus disease.
Another type of COVID-19 is highly sensitive and specific Molecular RT-PCR diagnostic test that is tested using Thermo Fisher Scientific’s equipment. This COVID-19 test was Authorized by the FDA under an Emergency Use Authorization (EUA) to detect the presence of SARS-CoV-2 Coronavirus in upper respiratory specimens from individuals suspected of COVID-19. All tests are performed by a U.S. licensed medical professionals in a CLIA certified laboratory.

What is the source of COVID-19?

The documentat of recovery from COVID-19 is an official letter from a licensed healthcare provider or a public health official stating that you have been cleared to travel. It must be accompanied by a positive COVID-19 viral test result obtained no more than 90 days before your return flight to the US. The timing can be different for other destinations.
How do I get the Document of Recovery form COVID-19 in order to travel to the United States with a positive test result?

Can people who recently recovered from COVID-19 or recently received a Positive COVID-19 test result travel?
Yes, people who have recovered from COVID-19 or have recently received a positive COVID-19 test result can return to the United States safely if they meet the CDC’s criteria for travel.
Travelers who have recovered from COVID-19 within the last 90 days do not need to get tested before or after their trip. If they develop COVID-19 symptoms before, during, or after travel, they should isolate and seek testing recommendations from a healthcare provider.
Those flying to the United States from another country can show documentation of COVID-19 recovery instead of a negative test result before boarding their flight.
When will I get the Document of Recovery form COVID-19?
If all requirements were met during the initial online submission, an official COVID-19 Document of Recovery will be emailed to you as a.pdf attachment within 12 or 24 hours, depending on the option selected prior to submitting the online form.
Do I need to make an appointment to get the Document of Recovery from COVID-19?
No appointment is required. Simply fill out and submit the Document of Recovery Online Form on our website.
What are the requirements for issuing the Document of Recovery from COVID?
According to the CDC’s Travel guidelines, the following criteria must be met to get the Document of Recovery from COVID-19:
Criteria for self-isolation have been met
Copy of your positive COVID-19 test result
COVID-19 test type – must be either a PCR, RT-PCR or an Antigen
COVID-19 test result – must show a Positive result
COVID-19 test specimen collection date
Name and contact information of the laboratory, healthcare facility or healthcare personnel who performed the test
Personal identifiers – full name, date of birth, age, passport number or driver’s license.
COVID-19 symptom dates (if the positive test was received in the last 10 days)

What should I do if I haven't received my Document of Recovery from COVID-19 in 12 or 24 hours?
The most common reason for not receiving the document of recovery is that the email was filtered into your junk or spam folder. Please look in your spam/junk folder for your official document.
The second reason for the document not being received is that your submission requires more information. In this case, a member of the medical team will contact you within 12-24 hours (by email or phone). Please monitor your messages.
