The documentation of recovery is an official letter from a licensed healthcare provider or a public health official stating that you have been cleared to travel. It must be accompanied by a positive COVID-19 viral test result obtained no more than 90 days before your return flight to the US. The timing can be different for other destinations.
Eligibility
You may be eligible for a Document of Recovery from COVID-19 for travel to the US if the following conditions are met:
- Criteria for ending self-isolation is satisfied and
- Proof of a recent COVID-19 infection is provided with the following information:
– a positive COVID-19 PCR or a positive COVID-19 Antigen test result
– specimen collection date is within 90 days
(Note – results from at-home tests (aka OTC or self-tests) will not be valid)
– The test result includes information that identifies the person
– The test result includes name and contact information of the facility or health care professional who performed the test
– The test result is Positive

What you need to know:
Individuals who have recovered from COVID-19 can continue to test positive for up to 3 months after their COVID-19 infection
Individuals who had a Positive viral (Antigen or PCR) SARS CoV-2 test may travel with the positive test result with the following conditions:
- Sample taken during the past 90 days and
- Test result is positive and
- Have a signed letter from a licensed healthcare provider or a public health official stating that you have been cleared for travel or clear the person to end isolation
- Must be in accordance with the CDC’s Guidelines
The test result must show information where the test was performed, including the name and contact information for the laboratory or healthcare personnel who performed the test.
The information that identifies the person must be included in the test.
Example: identifiers could include, but are not limited to, the person’s name, date of birth, and passport number.
The Documentation of Recovery will include:
- The clinic’s name, address, contact information, and license number
- Your identifying information: full name and date of birth that matches your passport or other travel documents
